1. try a newer version, like .6-something :)
2. Look in the theme.rc for the theme.
It will DL the module from loose-screws.com or shellfront.org, the filename minus the .zip extension.
That's assuming it's Omar's installer (3.02 has lines for loose-screws and shellfront, IIRC) and a OTS2 theme.
After that, if it loads it, read the docs for the module itself.
